Incident report form
An incident report form captures the facts while they are fresh: what happened, who was involved, what was done straight away and who owns the follow-up. Use it for injuries, near misses, equipment failures, food-safety issues and customer complaints.
When to use it: Complete it as soon as possible after the incident, by the person who saw it or the manager on duty.
